  • Instructions for filling in and sending the Card Transaction Dispute Form

To submit a Card Transaction Dispute Form, please follow the steps below:

  1. Download the application in pdf or word form.
  2. Fill in the application form, following the instructions below:  

     -Fill in the card account number with which the disputed transaction was completed.

    -Fill in your full name and your contact details.

    -Fill in the details of the disputed transaction. In case there is not enough space on the form to declare the disputed transactions, please attach relevant list.

    -Select a type of dispute, attaching the required supporting documents.

    -In case you select category 1, you must be aware that a prerequisite for the processing of your dispute, is the cancellation of your card.

    -Fill in the dispute submission date and sign the form.
  3. Send the form to, attaching the required supporting documents.
